HIPPA
The Health Insurance Portability and Accountability Act (HIPAA) of 1996 is a federal law designed to protect the privacy of protected health information. Protected health information, as described in the HIPAA Privacy Rule is: individually identifiable protected health information that is a subset of protected health information, including demographic information collected from an individual and that (a) is created or received by a covered entity and (b) relates to the past, present or future physical or mental health or condition of an individual; and (c) that identifies the individual or with respect to which there is a reasonable basis for believing that the information can be used to identify the individual.
4MOST Health Network strives to protect the privacy of protected health information of current and former customers. We want you to know that the information is safe and used responsibly and in accordance with the HIPAA privacy rule.
At 4MOST, we have administrative, technical, and physical safeguards in place to ensure the privacy of your personal health information. These include:
- Policies and procedures for handling protected health information
- Limited access to facilities where protected health information is stored
- Requirements for third parties and vendors to comply with privacy and
security practices
- Continuous review of company privacy and security practices.
In addition, we provide HIPAA privacy and security training to all employees to ensure that employees are dedicated to keeping your personal health information safe and secure.
In repricing medical claims, which contain personal health information, we obtain personal health information from health care providers, insurance companies, and third party administrators (for claims information). We also obtain personal health information from health care providers for credentialing purposes. As part of treatment, payment and health care operations, in accordance with the HIPAA Privacy Rule, we may use and disclose personal health information about you to reprice your medical claim or to forward it to an insurance company or third party administrator for processing.
